Please choose only one of the following above as we only need one form of ID. The reason we have to do this is that we need to identity you are the paying account holder. Feel free to black out, or erase anything sensitive information if you are concerned about security issues as we only require your full name and address. Sending us a scanned photo of yourself is not acceptable.
STEP 2 - Upload Your Identification
If you need information on how to do this, we recommend the following options below to provide this type of identification to us.
1). Scan the file to your PC.
2). Fax your ID.
3). Take a picture of your ID using any type of camera.
4). If you have an online account, or your ID is on your PC, you can also press CTRL, ALT, PRT SC which copies your PC screen, and then go into paint and paste an image, and then save it.
STEP 3 - Sending Your Identification
1). Attach the file to this email, and send it back to us.
2). Fax your id to the following fax number +1 (312)212-5885
3). Upload your id to a website and send us the URL link
4). Request to text or upload your information to +1 (312) 212-5885
Once your identification has been sent, we will verify this, and then process your order. We appreciate your cooperation in this matter at this is a security measure to protect all payment account holders.
Remember, this is an ONE time verification. Once you send the ID, if our system auto replies to you then that is confirmation that we have received your ID.
